The three conditions that must ALL be met to use the 16-hour window extension. No mileage radius is required.
A driver may extend the 14-hour period to 16 hours if: (1) the driver has returned to the normal work reporting location and been released for the previous five consecutive duty tours; (2) the driver returns to the normal work reporting location and is released from duty within 16 hours after coming on duty following 10 consecutive hours off duty; and (3) the driver has not used this exception in the previous 6 consecutive days, unless the driver has begun a new 7- or 8-day period following a 34-hour restart.

Establishes the 11-hour driving limit and 14-hour on-duty window (extended to 16 hours by § 395.1(o)) that apply regardless of where the driver travels.
No motor carrier shall permit or require a property-carrying driver to drive more than 11 hours following 10 consecutive hours off duty, or after the end of the 14th hour after coming on duty following 10 consecutive hours off duty.

Taking 34 or more consecutive hours off duty resets the 7/8-day cycle and also resets availability of the § 395.1(o) 16-hour exemption.
Any period of 34 or more consecutive hours off duty shall be deemed to end a driver's prior 7- or 8-consecutive-day period and begin a new period for purposes of this section.
